Do you have a valid state Driver's License?*

Are you willing and able to lift a minimum of 40 pounds?*

I understand this is a mobile job in which I drive my own vehicle.*

Are you at least 18 years of age?*

Are you available to work weekends?*

Are you available to work Monday through Friday from 9 am until 4 pm?*
